Whenever you add input elements, be sure to either add them with a corresponding label element (which you can visually hide). Or you can add the aria-label attribute to the input elements. This means that screen reader users will know what the form field is for.
You've done a great job getting the layout looking like the design, but I'd definitely say it could undergo one more round of refinements (font-size, font-weight, etc) to make it look as close to the design as possible.
